Allergic Disease and Asthma Center is administering the Moderna COVID-19 vaccines. The FDA requires that everyone who receives a COVID-19 vaccine receive and review a fact sheet.

On the day of your vaccination

If you have a health insurance card, please bring it. But, remember, there's no charge to you for the COVID-19 vaccine. In addition, identification is not required.  
Please do not arrive more than 15 minutes before your scheduled appointment.
